We now accept Medicare Part B, Anthem Blue Cross/Blue Shield insurance, Connecticare and Aetna.

Participants must bring their insurance cards to the clinic for billing purposes.

TO SPEED UP APPOINTMENTS, REMEMBER:

  • Fill out your 2012 Flu Shot Permission Form before you arrive. (We will have them available on site also, if you forget to bring it.)
  • Arrive ten minutes before your scheduled time
  • Wear clothing with loose sleeves or short sleeves

AND ALSO

  • Check with your doctor if you have a serious egg, chicken, or thimerosal allergy, may be pregnant, are ill with fever, or have/had Guillain-Barre Syndrome.
  • No one will be refused vaccination due to inability to pay.
